St. Eustache Minor Hockey Association
MEDIA RELEASE & CONSENT
At time of Registration, parents/guardians of participant registrants have the option to grant permission to St. Eustache Minor Hockey Association (STEMHA) to video record, audio record or photograph their child’s name, image, and voice (collectively referred to as “Records”) and to display, publish or distribute these Records for the purpose of publishing, posting on the STEMHA website, social media sites, for display inside the St. Eustache Arena and/or for broadcasting on television, radio or newspapers as determined by STEMHA.
By providing permission, parents/guardians waive any right to approve the use of these Records now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Records.
By providing permission, parents/guardians understand that the Records may appear in electronic form on the internet or in other publications outside of STEMHA's control, and agree that they will not hold STEMHA responsible for any harm that may arise from such unauthorized reproduction.
